Kannon Health currently offers Remote Patient/Health Monitoring (RPM) on the following physiologic data points.

BLOOD PRESSURE
Remote blood pressure monitoring allows patients to keep track of their blood pressure from the comfort of their own homes. This convenient method ensures consistent monitoring, early detection of potential health issues, and better management of hypertension. By using automatically linked blood pressure monitors, patients can easily share their readings with healthcare providers in real-time, facilitating timely medical interventions and personalized treatment plans.

Weight
A remote digital scale offers a convenient and effective way to track weight loss progress from home. This ensures regular and accurate measurements, enabling individuals to stay accountable and motivated. By using a remote scale, users can easily share their weight data with healthcare providers or personal trainers in real-time, facilitating personalized guidance and timely adjustments to their weight loss plans.

Oxygen Levels
Using a remote pulse oximeter to monitor oxygen levels provides a reliable and convenient way to ensure respiratory health. Tracking their blood oxygen saturation and heart rate from home, offers peace of mind and early detection of potential issues.
Remote monitoring with a pulse oximeter is especially beneficial for patients with chronic respiratory conditions, improving disease management, reducing hospital visits, and enhancing overall quality of life.

Initial Consultation
We meet with patients to answer questions, check insurance eligibility, and determine if this service is a good fit for the patient. Our amazing RPM team will be the ones to speak with our patients.

Remote Physician Visit
Patients will meet once with our preventive medicine physician, to gather relevant health background, discuss clinical goals and answer questions.

Device Set-Up and Education
At this meeting, our patients will receive their FREE blood pressure monitor, paid for by insurance. Our RPM team will handle all of the technology, set-up, and ensure patients feel comfortable with taking their readings. This can also be a remote virtual video visit.

Monthly Analysis and Check-In
Throughout the month, our RPM team will monitor your blood pressure values, alert patients for any concerning readings, and then create a monthly analysis of the patient's progress. Our RPM team will speak with patients at least once a month on the phone to go over the analysis. A PDF summary will also be available to share with the patient and other healthcare providers (as requested).
